ONLINE APPLICATION and CERTIFICATION PROCESS:
Complete 5 pages of the application form.
Submit payment in the amount of $150.00 USD.
You may save your application and return at anytime to complete it.
Your application will be reviewed by ICCDP. Upon approval you will be sent information on how to access the online Alzheimer's disease and Dementia Care online Course which is required if pursing CDP certification.
Register to take the course with Relias Learning and submit payment in the amount of $150.00 USD to Relias Relias. You have 30 days to complete the course work
Take the test.
You will be given multiple opportunities to retake the test should you fail the test.
Once you pass the test your certification will be emailed to you.
IMPORTANT: Do not proceed with the renewal process until you have completed 10 hours of continuing education.
Step 1: Complete 10 hours of continuing education. You are invited complete the 10 hours of continuing education with Relias Learning at http://iccdp.academy.reliaslearning.com/cdp-certification-renewal.aspx. Select the package that is created for the year that you are renewing. If you are renewing in 2019, please select the 2019 package when you take the online course.
Step 2: Complete CDP renewal application before expiration date.
The CDP renewal application cost is $150.00 USD. Payment by credit card. Late fee is $25.00.
Following completion of this form you will be sent to a page to submit payment by credit card. We accept Visa, Master Card, American Express, and Discover.
